Submit original passports of parents
Submit original and a copy of Birth Certificate issued by Ministry of Health duly attested by Ministry of Foreign Affairs.

Paste one recent photograph of mother (35 x 45 mm) on application and sign across with half the signature on the photograph and half on the application.
Attach 2 recent photographs of child
Husband's name must be endorsed in wife's passport before registration of child birth.

Husband's name must be endorsed in wife's passport before birth registration.

Submit prescribed deed for the change of name after marriage and/or addition of husband's name in her passport (Deed form available in the Embassy - Free of charge )
Submit marriage affidavit in English only signed by the wife (Affidavit available in the Embassy - Free of Charge)

Submit sworn affidavit for change of name of a minor signed by father or mother
Submit original passport.
Those born in Kuwait should first change their name on the
birth card issued by Ministry of Health prior to change of name on
passport. For the same a no-objection certificate can be obtained
from the Embassy for obtaining a new birth card with new name from
the Ministry of Health. (Fee for no-objection certificate is KD 6.000)
